Complex Case Manager
A fantastic opportunity has become available for a suitably qualified mental health professional (Social Worker/Occupational Therapist/Psychologist/Registered Nurse) working with a leading Government Department, based in Sydney CBD. The aim of this program is to provide specialised, goal-oriented case management to contemporary veterans and dependants identified as having complex and multiple needs. As a Clinical Support Case Manager you will manage your own caseload in order to ensure continuity of care of clients.
ABOUT YOUR ROLE:OVERSEE a caseload of clients that have complex needsPROVIDE clinical risk assessments for identified clientsMANAGE escalations to internal senior managers and external agencies where significant risk is identifiedCOMPLETE appropriate documentation regarding process, nature and outcomes of risk assessmentCOLLABORATE with staff members on request on areas of subject matter expertiseDELIVER case management for clients of the Client Support Framework
ABOUT YOU:Qualifications and/or relevant registration with AHPRA as a Psychologist, Registered Nurse (Mental Health),Occupational Therapist, or Social Worker (AASW).Working with Children Check & Vulnerable peoples checkEXTENSIVE experience within the clinical case management space (2 + years)Defence and/or Veterans Affairs knowledge (distinct advantage)
