Are you an experienced CHC Nurse Assessor looking for your next interim CHC contract in the North West? Are you looking to working as part of a supportive team, collaborating with other CHC professionals to share best practice? If so, a Continuing Healthcare team in the North West is looking for an experienced CHC Nurse Assessor for an interim contract completing supporting with a project clearing a backlog of CHC eligibility reviews across Bolton.

This role is suitable for CHC Nurse Assessor with a wealth of knowledge and experience of the National Framework.

This is an excellent opportunity for a Nurse Assessor who is comfortable and confident working autonomously, completing reviews electronically. This is a well organised team who offer the opportunity for all interim staff to shadow another member of the team to ensure you are fully setup and supported when starting your contract.

Full or part time hours are considered.
The successful candidate can expect attractive pay rates of £250-£270 per day

The responsibilities of the CHC Nurse Assessor will include:
- Completing reviews of patients in receipt of CHC/FNC funding, assessing change in their current health needs.
- Utilising extensive knowledge of CHC framework
- Providing advice to relatives and members of MDT in relation to CHC/FNC funding
- Completing written reports outlining recommendation of eligibility
- Submitting completed written reports in a timely manner

The successful CHC Nurse Assessor will be able to demonstrate:
- Thorough understanding of the CHC National Framework
- Highly motivated and able to work autonomously
- Excellent written and verbal communication
- Excellent people management skills

The minimum requirements for this role are:
- NMC registered
- Nursing qualification
