Homeless people are among the most vulnerable in our society. Recent research has shown that in Manchester nearly three quarters of those who are homeless have mental health problems. This role will be working for a Homeless Pathway Community Mental Health Team (CMHT) in Manchester City Centre.

You will work alongside other services to identify undiagnosed serious mental illnesses and provide assessments of the risks and needs of the service user to develop a referral pathway to suit their needs.

Responsibilities of the role:


* Developing care and recovery plans aimed at giving your patients the best quality of life.
* Develop referral pathways to suit patients needs.
* Discharge planning.
* Care Coordination.
* Running Depot Clinics.
* Detailed Note Taking.

* A valid and in-date NMC Pin.
* Mental Health Nursing Qualification (Diploma or Degree)
* Experience of working as a Community Mental Health Nurse at a Band 6 level.
