Attention Hamilton area Disability Case Managers! We have an amazing opportunity for you with a company who is established in the Hamilton city core. You will be responsible for assisting employees while they are off work or at risk of being off work for health reasons. You will assess situations and determine the best strategies to ensure a timely return-to-work process.

Your responsibilities will be, but not limited to:

• Manage Short Term Disability and workers compensation claims
• Conduct case assessments, communication with both employee and direct supervisor
• Reporting on situation outlining all factors why employee can or cannot return to work
• Depending on the situation you will provide the following interventions: in-depth assessment, medical management intervention, initiation and coordination of occupational or psychological interventions, etc
• Determine the appropriate return to work goal with both employee and supervisor. You will also support for the return to work planning process
• Communicate proactively with Human Resources regarding case management issues (if any). If necessary manage transition to long term disability.
• Attend case/disability management department meetings
• Other assignments or projects as assigned.

Qualifications
Each of the following qualifications must be met to be the successful candidate for this role:
• 5 years plus working in Disability Case Management experience working with short-term disability and worker's compensation claims
• 3 plus years plus Return-To-Work planning and facilitation
• NEER Rating and WSIB Appeals experience
• Knowledge of disability management and proceedures
• Most secondary diploma specializing in disability management and mental health
• Proficient in Case management and Microsoft office software (Word, Excel, Outlook, PowerPoint)
• Ability to learn quickly
