Are you an experienced Registered Nurse with a wealth of experience in Continuing Healthcare? Are you looking to work with a supportive team, where you will be valued for your commitment and hard work? If so, a CHC team in Harrogate is looking for an experienced CHC Case Manager for an interim contract reviewing packages of care across North Yorkshire.

This role is suitable for someone with a strong understanding the National Framework for Continuing Healthcare, who can work autonomously and hit the ground running to support the team. You will be working closely with families and other health and social care professionals, assisting with setting up and reviewing existing packages of care for individuals with complex health needs.

Full time hours preferred however 4 days per week will also be considered.
Daily rate of £280 - £300 per day.
3 month contract initially with potential of ongoing further.

The responsibilities of the CHC Case Manager will include:
- To recommend, plan, and procure safe and appropriate complex specialist care packages based on individuals needs
- To procure safe and appropriate care packages based on individuals needs
- To commission individual packages of care as per systems and procedures
- To case manage all fully funded CHC cases whether at home, in specialist or other care settings.
- To assist the Local Authority with jointly funded packages.
- Assist with complaints, IRP's and retrospective cases from a clinical view point.
- Conduct ongoing reviews.
- Ensure any safeguarding or examples of poor quality care are appropriately responded to reported and followed through.
- Undertake making emergency placements should the need arise.

The successful CHC Case Manager will be able to demonstrate
- Strong understanding of the CHC National Framework
- The ability to produce an informative and detailed DST assessment
- Highly motivated and able to work autonomously
- Excellent written and verbal communication
- Excellent people management skills with the confidence to work with individuals with varying clinical conditions

The minimum requirements for this role are:
- NMC registered
- Nursing qualification
