Are you a nurse who is dedicated to making sure patients receive safe, good quality care in the community? If so, a Continuing Healthcare team in Durham is looking for a talented Nurse Assessor to join the team, completing CHC assessments around the County Durham.

In this role the CHC Nurse Assessor can expect excellent benefits, including competitive pay rates £17 - £24 per hour, flexible working hours and free parking in a great location. The team are happy to consider part time and full time hours, although full time (37.5hrs) is preferred.

The responsibilities for this role include:

- Meeting with families and patients to conduct reviews of CHC and FNC eligibility.

- Conducting reviews on an annual and three-monthly basis, and writing reports around findings

- Making recommendations to appropriate CCG's and CSU's, and writing up cases to be presented at panel meetings

- Joint working with local authority teams to discuss any joint-funded applications/reviews

- Arranging and taking part in Continuing Healthcare assessments with other members of an MDT

The minimum requirements for this post are:

- NMC registered

- Nursing qualification
