Responsibilities:



* Full support for clients
* Help assess and move clients into more independent living
* Manage care and support packages
* Liaise with local authorities and professionals to ensure client care is person-centred and progression based
* To work in line with CQC guidelines including completing relevant training and certifications

Requirements:



* Full-time availability
* Experience coordinating and moving on vulnerable people into independence
* Portable clean DBS preferred
