Are you an experienced Mental Health Nurse, passionate about ensuring patients are receiving the best quality of care to meet their individual needs? Are you looking for an interesting and varied interim post that offers flexible working? If so, I'm working with a CHC team in Durham who require and Inpatient Nurse Coordinator to work across Durham, Darlington and Tees.

The post holder will complement the work of the CHC team by providing case management oversight and coordination for patients under the Mental Health Act and who are in either Mental Health or Learning Disabilities Trust or independent hospital provision.

Full or part time hours are considered.
The successful candidate can expect attractive pay rates of £24.00 - £28.00 per hour

The responsibilities of the Inpatient Nurse Coordinator:
- Assessing the healthcare needs of individuals with complex mental health conditions and learning disabilities.
- Provide clinical expertise as part of MDT
- Identified individual health outcomes as part of care planning process to ensure care package is beneficial to clients needs
- Reporting possible safeguarding issues
- Attend Mental Health Tribunals, Court of Protection, CPA's and Best Interests meetings
- Monitor and review cases to ensure their eligibility of care is appropriate and the package meets the clients needs
- Attend regular clinical supervision sessions to share good practice
- Provide detailed financial breakdowns in relation to individual funding requests

The successful Inpatient Nurse Coordinator will be able to demonstrate:
- Ability to lead and facilitate MDT meetings
- Experience reviewing health and nursing care needs of individuals with complex mental health issues.
- Experience of writing comprehensive reports to tight deadlines
- Highly motivated and able to work autonomously
- Excellent written and verbal communication
- Excellent people management skills

The minimum requirements for this role are:
- NMC registered
- Nursing qualification - RMN
