A fantastic role as a Social Worker has just become available in Manchester Councils Integrated Discharge Team. The contract will be for 6 months with the possibility of extension. Along with progressing your career we can offer an hourly rate of up to £26.88 per hour Ltd.

The successful candidate will be responsible for:

-Working in an acute General Hospital discharge setting
-Directly working with service users and carers to assess needs, assets and risks to independence
-Producing clear and analytical assessments which identify eligible needs of service users
-Working independently, while planning & prioritising caseloads
-Developing, monitoring and reviewing independence / support plans that reflect service user and carer choice and control, promote and maximise independence, social inclusion and personal responsibility, and continue to meet eligible needs

In order to be considered for this role, you must:

-Good knowledge of the Care Act 2014 and strength based assessment processes
-Access to your own vehicle and upto date business car insurance
-Have minimum 2 Years experience within a Hospital discharge setting
-A Degree/Diploma in Social work
-HCPC registration & valid DBS
