We are looking for a social worker to join the Adult Social Care Team, which is committed to providing high-quality support and services to individuals and families in Bracknell Forest. As a Social Worker in the Busy Hospital Team, you will play a crucial role in facilitating timely and effective hospital discharges while promoting the well-being and independence of our service users.
Responsibilities:
- Work collaboratively with hospital staff, multidisciplinary teams, and community resources to facilitate safe and timely discharges for patients.
- Conduct Mental Capacity Assessments to determine individuals' ability to make decisions regarding their care and support.
- Utilize a strength-based approach and the conversations model to engage with service users, identify their strengths and needs, and empower them to connect with their community and social networks.
- Apply problem-solving skills to address complex issues and barriers to discharge, working closely with service users and their families.
- Provide support and advocacy for vulnerable adults, including those at risk of abuse or neglect, and implement safeguarding measures as necessary.
- Maintain accurate and up-to-date documentation of assessments, interventions, and support plans in accordance with professional standards and regulations.
Requirements:
- Bachelor's or Master's degree in Social Work from an accredited institution.
- SWE registration as a social worker.
- Proven experience in hospital discharge planning, Mental Capacity Assessments, and safeguarding within a social care setting.
- Valid driving license and access to a car for travel between the office and hospital locations.
