Job Title: Adults Social Worker (Prevention Team)
Location: Newcastle upon Tyne, Tyne and Wear, UK
Salary: £20 - £24 per hour
Newcastle City Council is seeking a dedicated and compassionate Adults Social Worker to join our Prevention Team. As an essential member of our team, you will play a crucial role in supporting adults in our community to live independently and improve their overall well-being.
Responsibilities:
- Conduct thorough assessments of adult's needs, strengths, and risks to develop comprehensive support plans tailored to individual circumstances.
- Coordinate and facilitate access to a range of support services and resources, including health-care, housing, financial assistance, and social activities, to meet the identified needs of service users.
- Advocate on behalf of service users to ensure their voices are heard and their rights are upheld within various settings, including liaising with health-care professionals, housing agencies, and legal representatives.
- Identify potential risks to the safety and well-being of service users and implement effective risk management strategies in collaboration with relevant stakeholders.
- Provide timely and responsive support during crisis situations, including safeguarding vulnerable adults from abuse or neglect, and coordinating emergency assistance as necessary.
- Empower service users to develop and maintain their independence through the provision of practical support, skill-building activities, and access to community resources.
- Work collaboratively with colleagues within the council, as well as with external partners such as health-care providers, voluntary organisations, and other statutory agencies, to ensure integrated and holistic support for service users.
- Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with relevant policies and procedures, and prepare timely reports as required.
Qualifications:
- Registered with Social work England
- A Degree in Social work or Diploma in SW, CQSW, CSS
- Right to Work
- Enhanced DBS check, dated within the last 12 months
- Experience with Adult and Older People Services, including familiarity with challenges related to Section 21A of the National Assistance Act 1948.
- Experience in Deputyship work, demonstrating an understanding of the legal and practical responsibilities involved.
- Ability to work independently and as part of a multidisciplinary team.
