Sutton is seeking a dedicated and experienced Qualified Social Worker with a caring and calm approach for an exciting and rewarding opportunity within our Adult Services Transition Team. This role focuses on supporting young people with additional needs and their families as they transition to adulthood. Working closely with Children's Services, Education, and Health, you will ensure that transitioning young adults receive the right support from the right people.
Key Responsibilities:
... - Assessment and Support Planning: Conduct Care Act 2014 compliant eligibility assessments for young people and their carers. Develop person-centred, strengths-based support plans for high-cost placements and care packages.
- Continuing Healthcare: Complete comprehensive Continuing Healthcare (CHC) checklists and collaborate with NHS partners to secure CHC funding for eligible young people.
- Monitoring and Review: Regularly update support plans to reflect the changing needs of young people, ensuring they provide good value for money while advocating for their rights and access to services.
- Safeguarding: Promote the welfare of young people, adhering to relevant legislation and policies. Identify and assess levels of risk and need, undertake safeguarding investigations, and deliver protective/supportive services.
- Case Management: Manage a complex and varied caseload, making sound professional judgements with appropriate supervision, guidance, and support.
- Collaboration: Work with multidisciplinary teams, including children's services, education, healthcare professionals, and community partners, to adopt a holistic approach to meeting the needs of young people.
Requirements:
- A Qualified Social Worker with a Social Work Degree or equivalent
- A valid SWE registration
- Ability to attend the office in Sutton 2 times per week
