The Role
As a Social Worker within the Let's Talk Team, you won't just be ticking boxes. You will be at the forefront of our "Let's Talk Together" approach.

Empower Individuals: Undertake person-centred assessments that promote health, wellbeing, and independence.
Collaborate on Complexity: Work with people with complex needs to formulate support plans that maximise their abilities and reach their personal goals.
Community Connection: Become an expert on the local community, linking people with universal services and non-care support to foster resilience.
Promote Choice: Proactively embed personilisation into your practice through individual budgets, direct payments, and self-directed support.
Key Responsibilities
Move away from "service-driven" models toward outcome-focused support that utilises an individual's own strengths and family/community resources.
Ensure the "Making Safeguarding Personal" ethos is at the core of everything you do, balancing risk management with personal liberty.
Take professional responsibility for a diverse caseload, providing high-quality written reports and maintaining accurate data.
Contribute to the development and quality assurance of our Integrated Care Services, ensuring we remain responsive to both local and national landscapes.
Engage in regular reflective supervision (EPM), maintain HCPC registration requirements, and provide support to less experienced colleagues.

Requirements: You must hold a degree in Social Work and be registered (or eligible for registration) with Social Work England.
