Are you a resilient Social Worker who thrives in a high-energy environment? We are looking for a dedicated professional to join our Unscheduled Care team. This role is at the heart of the Home First approach, where your primary mission is to prevent unnecessary hospital admissions and ensure patients receive the right care in the right place-ideally, their own home.
Based primarily at the East Lothian Community Hospital, with occasional travel to the Royal Infirmary of Edinburgh, you will be a key player in a daily Multi-Disciplinary Team (MDT) process.
Your Responsibilities
Rapid Response: Deliver short-term, high-impact interventions to stabilise complex situations quickly.
Admission Prevention: Work alongside A&E and acute ward colleagues to "turn people around" at the front door, preventing avoidable admissions.
MDT Collaboration: Respond to referrals screened by the Team Manager and collaborate closely with health and social care partners.
Agile Casework: Navigate a fast-paced referral system that requires quick thinking and decisive action.
What You Bring
Experience: Proven background in Adult Social Work, ideally within a hospital or unscheduled care setting.
Agility: The ability to adapt to a changing environment and manage a fluid caseload.
Mobility: A valid driving licence and access to a car are essential for this role due to the rural geography of East Lothian.
Technical Skills: Experience with MOSAIC is a significant advantage, though not essential.
