Randstad are looking for a health and social care practitioner to be a part of a team to fulfil a pilot project that focuses on future accommodation and care needs for young people going through transition.Job details:The role requires a health and social care practitioner to work in a team to fulfil a pilot project.The pilot project will focus on ensuring early and detailed planning for transitioning for individuals.Specific focus will be on to accelerate planning to ensure the needs and outcomes of identified young people aged between 16 and 17 where there is a consideration of accommodation and care required post 18.The pilot will look at options for new innovative accommodation models.
